Urgent Care

What should I do if I do not feel well and your office is closed?

For emergencies, please contact University Police at (934) 420-2111.

For non emergent matters after hours, we have linked the following urgent care centers local to our campus:

If I feel sick, can I just go over the Health and Wellness Center?

Yes! While we prefer that students schedule an appointment, if you’re feeling sick and it’s urgent, you are welcome to walk in during our regular hours to be seen. For non-urgent visits, please use the Student Patient Portal to schedule an appointment at your convenience: . 

Covid-19

What do I do if I test positive for COVID-19?

If you test positive for COVID-19, stay home until you are fever free for 24 hours without the use of fever reducing medication and your symptoms are resolving.  

Once your symptoms have improved or resolved and you are fever free for 24 hours without the use of fever reducing medications, you can return to school and/or work. 

You may take additional measures for the next 5 days to curb disease spread, such as enhancing hygiene practices, wearing a well-fitting mask and keeping a distance from others.  These additional measures are encouraged.

What do I need to be in compliance with my health records?

Can I register for classes before I submit my immunization requirement?

No. Students must submit all required immunization documentation before they are eligible to register for classes.

To avoid delays, please upload your records as soon as possible through the Student Patient Portal at

All immunization documentation must be signed or stamped by your healthcare provider to be accepted.

When are my immunizations due?

You must submit the following before you can register for classes:

  • Proof of two (2) MMR immunizations (Measles, Mumps, and Rubella)
  • Proof of a Meningitis vaccination within the past 5 years
    OR a completed Meningitis Response Form
    *Resident students are required to submit proof of the Meningitis vaccination within the last 5 years.  A waiver will not be accepted for resident students.*
  • Please upload your immunization records through the Student Patient Portal at .

All immunization documentation must be signed or stamped by your healthcare provider to be accepted.

Immunization Requirements for Part-Time Students (Includes summer & intersession students taking up to 11 credits)

As a part-time student, you are still required to submit the following immunization documentation:

1) Proof of Two (2) MMR Vaccinations.

  • The first dose must have been administered on or after your 1st birthday.

2) Meningitis Vaccination Within the Last 5 Years OR a completed Meningitis Response Form.

  • If you choose not to receive the meningitis vaccine, you must indicate this on the Meningitis Response Form.
    The form can be found under Important Forms on our website.

Please submit your documentation through the Student Patient Portal at

Immunization & Health Requirements for Full-Time Commuter Students (Students enrolled in 12 or more credits)

If you are a full-time commuter student, you are required to submit the following:

1) Proof of Two (2) MMR Vaccinations.

  • The first dose must have been administered on or after your 1st birthday.

2) Meningitis Vaccination Within the Last 5 Years OR a completed Meningitis Response Form.

  • If you choose not to receive the meningitis vaccine, you must indicate this on the Meningitis Response Form.
    The form can be found under Important Forms on our website.

3) Recommended: A Complete Health Record

  • Includes immunizations, laboratory testing, and a physical exam. The complete health record form can also be found under Important Forms.

All immunization documentation must be signed or stamped by your healthcare provider to be accepted.

Immunization Requirements for Resident Students

If you are living in campus housing, you are considered a resident student and are required to submit the following immunization documentation:

1) Proof of Two (2) MMR Vaccinations.

  • The first dose must have been administered on or after your 1st birthday.

2. Meningitis Vaccination Within the Last 5 Years

This is mandatory for all resident students. Waivers are not accepted — you must provide proof of receiving the meningitis vaccine within the past 5 years to be eligible for on-campus housing.

Health Requirements for Student-Athletes

If you are participating on a 51³Ô¹Ï athletic team, you must complete the following health requirements before participating in any practices or games:

1. Complete Health Record

Must Include: 

  • Immunization Records (must be signed or stamped by your healthcare provider to be accepted.)
  • Physical Exam
  • Meningitis Response Form

*Returning athletes: An updated physical is required each year*

2. Cardiac Athletic Questionnaire

  • Required for all student-athletes
  • Any "Yes" responses will require clearance from a cardiologist before participation

3. Sickle Cell Trait Status Verification Form

  • Required for all first-year and transfer student-athletes
  • Must Include: 
    • The completed form
    • A copy of the lab results or newborn screening (HgbAS)

Health Requirements for Nursing, Dental Hygiene, and Medical Laboratory Technology Students

If you are enrolled in Nursing, Dental Hygiene, or Medical Laboratory Technology programs, please ensure the following:

1) Completion of a Health Record:

  • A complete health record is required every year. The health record should be dated on or after May 15th to be valid for the upcoming academic year. 

2) Submission of Titers with Lab Reports for: 

  • Measles
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B

3) Submission Process:

  • All forms and documents for Nursing and Medical Laboratory Technology students must be submitted through Castle Branch. Additionally, copies must be uploaded to the Health & Wellness Center via your
  • Dental Hygiene students must submit their records to the Health & Wellness Center every year.

All immunization documentation must be signed or stamped by your healthcare provider to be accepted.

Immunization Requirements for International Students

If you are an international student, please note:

  • Immunization records must be submitted in the original language along with a certified English translation.
  • A Tuberculosis (TB) Screening is also required and must be submitted directly to the Health & Wellness Center. The Tuberculosis (TB) Screening form can be found in the Important Forms section on our website.

All documentation should be submitted to the Health & Wellness Center or uploaded directly to your student .

Requirements for Online-Only (Remote) Students

If you are an online-only (remote) student, you are required to submit:

1) A completed Meningitis Response Form

  • This form can be found in the Important Forms section on our website or completed directly via the student .

What is an MMR?

MMR stands for Measles, Mumps, and Rubella. It is a vaccine commonly given in childhood to protect against these three diseases. The vaccination consists of two doses, with the first dose administered on or after your 1st birthday and the second dose admistered between 4-6 years old.

Where Can I Get a Copy of My Immunization Records?

You can obtain your immunization records from:

  • Your pediatrician or healthcare provider (must be signed or stamped to be accepted)
  • Your high school
  • A previous college or university you attended
  • If you’re unable to retrieve your records, you may have blood tests (titers) done for MMR to confirm immunity. The complete lab report must be submitted to be accepted. 

How can I submit my records?

  1. The most effective way to submit immunization records is through the student patient portal at . This will allow the Health and Wellness staff to process your records in a timely manner.

  3. Email: wellness@farmingdale.edu.
  4. In Person: Bring your documents directly to the Health & Wellness Center. If the office is closed, you can use the secure mailboxes located at the front and rear of the building.
  5. Mail to: 51³Ô¹Ï, Health and Wellness Center, 2350 Broadhollow Rd, Farmingdale, NY 11735.
Please make sure your name and RAM number appear on all paperwork.

How can I verify that you received my records?

If you uploaded your records via the patient portal:


Log in to the portal and select the Required Immunizations and Forms tab on the left. Here, you can check your compliance status:

  • Compliant / Satisfied: Your submission is complete
  • Not Compliant: Your submission is incomplete
  • No Data: No document has been submitted
  • Awaiting Review: Your documents have been received and are being reviewed by our staff.

You can also send a secure message through the portal to confirm receipt of your paperwork.

If you faxed your records:
Please allow 48 hours for processing, then call the office at (934) 420-2009 to verify.

If you mailed your records:
Please allow 7 business days before calling (934) 420-2009 for verification.

Can I Get My Immunizations and Health Record Completed at the Health & Wellness Center?

Yes! You can have your immunizations and health record completed right here at the Health & Wellness Center.
